Please complete and submit this online form in advance of your first visit, if possible—it helps us learn your pet’s medical history ahead of time so we can use our time together most efficiently.

No appointment is ever required for emergency care.

An appointment for emergency care at Mosaic Animal is never required, but if time permits, we encourage you to call our team at 505.416.5840 to discuss your pet’s condition and let us know you’re on the way. This helps us understand the situation and be ready for your arrival. 

Payment Information

When a member of our team discusses your pet’s care with you, they will also discuss the cost, so you know upfront exactly what’s involved. Payment is required when services are provided.

Mosaic SAVES Fund

If emergency care poses a financial hardship, it’s important to know that Mosaic Animal has launched a nonprofit arm of our organization that can assist pet parents in need – Mosaic SAVES.
